The General Radio 658 Tube Socket was introduced in Bulletin 933 (1931) and remained available through Catalog H (1935).
The Type 658 is a five-pin vacuum tube socket designed for UY-type tubes, a porcelain version of the bakelite Type 438. It is a surface mount design with solder tabs and screw type connections. Looking closely at the top, one can see the socket is labelled with triode tube elements.
